V4 patient orders search.

Returns a flat list of PatientOrderResponseV4 wrapped in
CommandOkResponse (shape: { "result": [ ... ] }, no paging/total-count envelope).
The V4 payload is lean: policy and encounter references are scalar IDs (PrimaryPolicyId,
PatientEncounterId, etc.), with only TherapyType / PatientOrderStatus / PatientOrderType /
UnitOfMeasure enriched inline and PatientEncounter enriched only when
IncludePatientEncounter is set — it is not the heavier nested V3 shape.
